Knee osteoarthritis affects more than 14 million Americans, and many of them eventually need total knee replacement surgery to ease pain and restore function. Opioids remain the main treatment for pain after this surgery.

What they work on
- Test whether cannabidiol (CBD, as the FDA-approved drug Epidiolex) taken around surgery reduces opioid use in the 4 weeks after total knee arthroplasty, in a double-blind, placebo-controlled trial of 380 patients.
- Separate the effects of CBD given one week before surgery from CBD given 4 weeks after surgery, using a 2x2 factorial design that randomizes each treatment period independently.
- Measure whether CBD reduces pain, anxiety, sleep disturbance and inflammation around knee replacement surgery.
- Characterize the side effects, adverse events and pharmacokinetics, meaning how the body absorbs and clears the drug, of CBD across the treatment course.
More about the research
However, about 80% of surgical patients report at least one moderate or severe side effect, such as nausea, dizziness or constipation. Opioid use around surgery is also linked to long-term dependence, opioid use disorder (addiction), overdose and death. Safer ways to control pain after surgery are therefore urgently needed.
Why it matters
Patients recovering from knee replacement could benefit from a pain medicine that lowers their need for opioids and the risk of side effects, long-term dependence, addiction and overdose that comes with them. With more than 50 million surgeries each year in the United States, evidence about CBD as an opioid-sparing medicine could reach well beyond knee surgery and guide postoperative pain care.
