More older adults than ever are living with traumatic brain injury (TBI), both from new injuries late in life and from injuries earlier in life they have survived into old age. Injury mechanisms and later effects differ markedly in older people, yet clinical guidelines for TBI offer no specific advice for them.

What they work on

  1. Identify patient characteristics that predict the share of time older adults spend in different health care settings during the first year after post-acute TBI care admission, using Medicare claims linked to the National Trauma Databank.
  2. Compare function, health, health care use and cost among older adults with remote TBI, later-life TBI, and no TBI, using Health and Retirement Study data linked to Medicare claims.
  3. Link the TBI Model Systems National Database to Medicare claims by probabilistic matching, building a resource to relate rehabilitation care use to long-term patient-reported TBI outcomes.

More about the research

Hospital registries and TBI cohort studies also track little about care after patients leave acute and rehabilitation settings. So the long-term care needs of older adults with TBI are poorly understood.

Why it matters

Older adults with traumatic brain injury, whether recent or from decades earlier, lack age-specific care guidelines. Describing their health, function, care pathways and costs from large linked datasets could inform age-appropriate models of care and rehabilitation. The linked Medicare and TBI Model Systems resource could also support future studies of real-world outcomes for people living with TBI-related disability.
