Metastatic prostate cancer is becoming more common, and only about 29% of men survive long term. The standard treatment for metastatic castration-sensitive prostate cancer (mCSPC) blocks male hormones.

What they work on
- Test whether 52 weeks of creatine monohydrate plus home-based telehealth resistance training preserves muscle mass better than placebo plus training in 200 men with metastatic castration-sensitive prostate cancer on androgen deprivation therapy.
- Compare the effects of creatine plus resistance training versus training alone on fatigue, physical function, independence, insulin sensitivity and quality of life at 24 and 52 weeks.
- Assess whether creatine supplementation produces favorable changes in markers of cancer progression, prostate-specific antigen (PSA) and cell-free DNA (cfDNA) in the blood, during the randomized trial.
More about the research
This approach, called androgen deprivation therapy (ADT), is combined with an androgen signaling inhibitor. The hormone blockade also wastes muscle. Loss of muscle is linked to shorter survival, fatigue, weaker physical function, loss of independence, poorer blood sugar control and lower quality of life. Resistance training (strength exercise) helps men with non-metastatic disease, but little is known about men with metastatic cancer.
Why it matters
Men living with metastatic prostate cancer on hormone therapy lose muscle, which harms survival and daily life. A low-cost strategy using inexpensive creatine and elastic bands, delivered at home by telehealth, could help preserve muscle, function and quality of life. It could also be adopted widely by institutions with existing telemedicine systems.
