Young people with type 1 diabetes (T1D) must eventually move from a pediatric clinic to adult care. Around that time they also start running their own health care.

What they work on
- Develop implementation strategies for a virtual T1D transition program using implementation mapping grounded in the COM-B model, which explains behavior through capability, opportunity and motivation.
- Pilot a randomized trial assigning 96 adolescents and young adults with type 1 diabetes to a virtual program of peer mentoring, health-system education and a written transition plan, or to a control group.
- Survey a nationally representative sample of pediatric endocrinology practices to measure which T1D transition resources exist, how many patients they reach, and the barriers to scaling the program.
More about the research
Many miss routine visits during this shift, and the gaps can worsen their health and raise emergency care use. Programs that ease the transition work, but they are often costly, hard to reach, and depend on big changes by clinics or health systems. This project develops a lower-cost, patient-centered program that adolescents and young adults (AYAs) can join online.
Why it matters
Adolescents and young adults with type 1 diabetes often fall out of care when they leave pediatric clinics, which can lead to worse health and more emergency visits. A low-cost virtual transition program, along with national data on what clinics currently offer, could help health systems keep young people connected to diabetes care.
