Drinking and smoking often go together, and together they are leading causes of preventable death in the United States, with a heavy toll in rural America. About 60–70% of smokers binge drink or drink dangerously, and over 75% of people with alcohol use disorder (AUD) smoke.

What they work on

  1. Test whether rural smokers with alcohol use disorder who receive varenicline plus app-based contingency management submit more breathalyzer samples showing alcohol abstinence and non-heavy drinking days than those using a control app.
  2. Determine whether varenicline plus contingency management increases smoking abstinence and reduction, measured with NNAL, a tobacco breakdown product, during treatment and follow-up.
  3. Identify mediators of treatment response by tracking daily fluctuations in the three Addiction Neuroclinical Assessment domains with ecological momentary assessment embedded in the app.
  4. Determine whether biological sex interacts with treatment assignment to produce different changes in drinking and smoking outcomes.

More about the research

Rural residents often face long travel and high costs to get treatment. Fully remote telehealth care could remove those barriers. This project tests whether remote treatment can help rural heavy-drinking smokers cut back on both habits at once.

Why it matters

Rural Americans who both drink heavily and smoke are common, at high risk and poorly served by existing care. If the hypotheses hold, a fully remote program combining varenicline with a smartphone breathalyzer reward app could become an evidence-based telehealth treatment. Such a program could help these patients cut alcohol use and quit smoking without traveling for care.
